This happened to me as well, only the result was more catastrophic. I´m glad you caught yours in time.
Is there a list of common faults to look for stickied somewhere? If it´s not there, it should be.

The insert is threaded to make sure that the guide rail is in line with the cam gears and then locked in place with the allen bolt.  You will notice that it is the only bolt on all of the guide rails that attaches to the head allowing for an adjustment if the head is replaced.
