I'd pull the slave cyl to see what's going on.

Did you swap the slave cyl? If so, try a reverse bleed. Here's a demo from BMP:

Did you ever have any pressure in the slave cyl? It's also possible that the shift fork fell off the pivot & let the slave cyl blow the rod & piston out. Those pivots can break too. Also, the fork retaining clip could have slipped out. Could even be a bad release bearing or the wrong part number. There are several different thickness release bearings, it's possible you got the wrong kit.

Dave,

Thanks for the reply, I need to check all of these, it feels in my guy more mechanical than hydraulic. However, I hope like hell its as simple as replacing the MC and SC and it getting fixed. Everything inside of the housing while it was off was touched, either replaced or cleaned and greased and put back in with proper torque and best practices applied. I'll report back but any other ideas are always appreciated.

UPDATE FIXED...

So perhaps installed a little off or something but the throwout bearing was a little "loose" and off of the spline, so re greased everything and back together and for the first time in 6 years, she's back on the road!!
