Transmission bad. What happened?

So i thought the release bearing was bad based on bearing noise going away when clutch pushed in. Pulled trans. Release bearing tight. Spun input shaft and grindy grindy! Popped off shaft cover and bingo! Ball bearings missing and little plastic bits inside. This ever happen to anyone? Also, pressure plate fingers show a little bit of uneven wear. Should i replace plate too?

Never happened to me, but that's unusual for a Getrag. I'd replace the worn components.
