SMOG fail

So I went in for a Smog test and failed at 15 mph.
I used that online calculator and found my car is running a bit rich.
Plugs are about two years old and look good. Slight tan color. COP kit installed.
Installed new universal o2 sensor two years ago for smog. new cat 2 yrs ago to pass.
oil change before test
Also Im running a MarkD 91 oct chip. And I have ford injectors to install.
here's the #'s
15 mph
CO2 O2
14.9 .2
HC
max : meas
124 : 154 FAIL
Co
.78 : .35
No
815 : 1088 FAIL
What should I check for that might make it run rich? I put my MM to the Engine Coolant Sensor and it is within specs.
Is there a way to check the AFM?
Last test i put a new cat and this dropped the #'s drastically but I want to get this thing running right so I don't need a cat every two years!
Maybe I didn't get the cat hot enough?
TIA

Make sure the engine and cat are both hot before starting the test. Looks like you're high on hydrocarbons and NOx.
If it was warmed up when you did the test, you probably need a new cat and O2 sensor.
