Possible failed chain tensioner

There is plenty of instructions on how to replace a failing chain tensioner, but has anyone experienced an actual failure?

My 91 318is cut out on me yesterday. After removing the rocker cover, it was obvious that chain tension had been lost. The chain is loose and had smashed the plastic chain guard. It's still sitting on the cogs, so I doubt the chain is broken.

Has anyone known of this happen?
If so what damage did this cause to the engine?

Unfortunately if the chain is totally loose and the engine died, chances are the valves smashed into the pistons and the cylinder head is toast. Its time for a rebuild.

I'll be helping Ejectorbed with this project.
I agree that normally the valves would be gubbed when this happens, however, the bottom of the engine is still turning on the key.

Would we get away with refurbing/replacing the head Or, as with my experience with Alfa engines, would the crank need replaced/reground and new shells etc.
