Any ideas?

My thinking is, worst case its a cracked head, best case it just needs a HG/profile gasket and some sensors.

sounds like fuel starvation. Check the oil and oil cap. Is the oil milky? How is the coolant level?

What's the history behind the car before it started running bad? Any maintenance performed prior?

Blue smoke on startup usually indicates bad valves seals and white smoke while running is normally associated with a bad HG/cracked head. Pull your plugs and look at them.
